Our commitment:

 

The Oral surgery practice of Dr Pienaar and his team is committed to comply with all applicable privacy laws that govern how we collect, use, disclose and store your information. We are bound by the Australian Privacy Principles under the Privacy Act 1988 (Cth) and other relevant laws about how private health service providers handle personal information.

 

Types of information that we collect and store:

 

Personal information: information about an individual from which an individual can be reasonably identified.

 

Personal information we collect may include:

  • Your name and surname

  • Your address (home and postal)

  • Your date of birth

  • Your gender

  • Your email address

  • Your occupation

  • Your contact details (telephone numbers etc.)

  • Details of an emergency contact person and their contact details

  • Your Medicare number

  • Your Private health fund details and number

 

 

Health/”sensitive” information: Information regarding an individual’s physical and mental health and health service provided to an individual.

 

Health information we collect may include:

 

  • Name of your medical practitioner

  • Name of your dental practitioner

  • Your medical history

  • Your dental history

  • Medicines or drugs that you are allergic to or any other allergies

  • List of current medications and conditions

  • Any other medical, dental or mental health conditions like radiographs, study models, clinical images etc.

 

 

Collection:

 

Personal and health information is usually collected directly from you, but sometimes we may need to collect it from someone else (for example another health service provider).

 

We take all reasonable steps in accordance with the Privacy Principles under the Privacy Act 1988 (Cth) to ensure the safeguarding of this information from misuse or loss and from unauthorised access, modification or disclosure.

 

Purpose:

 

We collect your information for the purpose of providing you with health care and for directly related purposes. For example:

 

  • To maintain medical and dental records

  • For use by our multidisciplinary treating team

  • To liaise with health professionals, Medicare and your health fund

  • In an emergency where your life is at risk and you cannot consent

  • To manage our surgery, including for processes relating to accounts, risk management, safety and security activities and quality assurance and accreditation activities

  • For the education of our staff

  • For other purposes required or permitted by law

 

Disclosure:

 

We will not use or disclose your personal information to any other persons or organisations for any other purpose unless:

 

  • You have consented

  • The use or disclosure is for a purpose directly related to providing your with health care and you would expect us to use or disclose your personal information in this way,

  • We have told you  that we will disclose your personal information to other organisations or persons, or

  • We are permitted to do so by law

 

We may outsource information and data storage services which may involve storing information outside of Australia. Where we outsource our services we take reasonable steps in the circumstances to ensure that third parties, including organisations outside of Australia, have obligations to comply with all laws relating to privacy and confidentiality of your personal information.

 

Data Breach Notification

A data breach is the unauthorised access to, disclosure or loss of personal information. Breach events are not confined to online or electronic systems.

Access, Correction and complaints:

 

You have the right to access your personal information in your health record. This access might be by inspection of your clinical records or by copy of information. There will be no charge made for requesting information, but there may be fees levied to cover cost associated with the processing of this request or the copying of information.

You can also request an amendment to your health record should you believe that that it contains inaccurate information.
